Concepts of Care is proud to be the first home health provider in the State of Louisiana to medically manage and regulate patients on Coumadin/Anticoagulation therapy. We currently maintain a total of five certified anticoagulation nurses. Our home health agency is the only one in the State of Louisiana to be certified as an Anticoagulation Center of Excellence.
This program focuses on patient care management as part of providing holistic Home Health Care in Lafayette, LA. Led by healthcare professionals in the industry, they will be sharing their insights on multiple home health care topics and guidelines.
