Concepts of Care Home Health, Inc. is proud to be the first home health provider in the State of Louisiana to medically manage and regulate patients on Coumadin/Anticoagulation therapy. We currently maintain a total of five certified anticoagulation nurses. Our home health company is the only one in the State of Louisiana to be ACE certified as an Anticoagulation Center of Excellence.
